What’s the cheapest way to get booking and consultation forms on one bill?
You don’t need two systems. Plenty of UK booking apps include consultation forms, but some keep them for a dearer plan. The cheapest plans we found with both on 2 November 2026 are free, at Slotora and Slotivo, each with limits. Covered Beauty, which we make, includes forms from £9 a month, and SalonBooking from £9. Fresha includes them from £17.94 with VAT. Phorest keeps sending forms before a visit for its Grow plan, Timely for its Boost plan, and Vagaro charges £10 a month for them.
We make Covered Beauty, so read this knowing that. Every other figure links to the provider’s own page. This is what each provider says it does. We haven’t tested every form in every app.

Can I use consultation forms without a booking system?
Yes, on some systems. On Covered Beauty you can run forms on their own, with no treatments, hours or bookings set up. Pick the client and up to 10 forms, then email a private link or copy it into WhatsApp or a text. The link works for 14 days, only for that client, and needs no app or account. In the salon, hand them your phone or tablet, and once you’ve set a kiosk PIN it shows only their forms until someone from your team enters it.
If forms are all you want, there are other routes:
- Faces. Free to use, no card needed. Who it suits: Aesthetics clinics and healthcare practices that want 200+ insurance-verified consent forms, with prescribers and pharmacies in the same place.
- Insync Consent. Free on the App Store. Who it suits: Beauty and aesthetics practitioners who want insurer-approved consent forms and aftercare on an iPad.
- Jotform. Starter: free for 5 active forms and 100 submissions a month. Bronze: £29/month, or £24/month billed yearly. Who it suits: Anyone happy to build their own forms, with consent form templates among many others.

What should I check in any forms system?
Most comparison pages stop at yes or no. These six things make the difference once you use it every day.
- Does it go out before they arrive? The point is a client who’s filled it in at home, not on your couch with a pen. Check it goes out with the booking confirmation, and that someone gets chased if it isn’t done.
- Will regulars fill it in every time? A lash client in every three weeks won’t thank you for 40 questions on each visit.
- Is there a separate tick for health answers? Answers about health are special category data under UK GDPR, so you need a clear lawful basis. Read our guide to consent forms and UK GDPR.
- Where does the signed form end up? It should sit on the client’s record, with the date, the name they signed and the version they saw, and you should be able to print it for your insurer.
- Can you use your insurer’s wording? Ready-made forms are a starting point. Ours aren’t approved by any insurer, so if yours gives you wording, you’ll want to type it in.
- What happens to the forms if you leave? Ask how you get them out, and in what format, before you sign up. You’ll need them for years after the client’s last visit.
How do forms work on Covered Beauty?
Forms are on every plan, from £9 a month. There are 23 ready-made forms, for lashes, brows, nails, waxing, electrolysis, hair extensions, tanning, skin, aesthetics, massage and piercing, and a new-client medical history form. Each one’s a starting point to read through and make your own, or you can build your own from scratch.
Link a form to the treatments it covers and it goes out with the booking confirmation, with a reminder 48 hours before if it isn’t done. Book two treatments together and each form comes up once. A form filled in without a booking counts for any appointment in the next 30 days, as long as you haven’t changed it. Clients sign with their typed name, and the form lands on their record, ready to print or save as a PDF.
When we’re not the right choice. Clients sign by typing their name, not drawing it, so if your insurer wants a drawn signature, look elsewhere. We can’t keep before-and-after photos in the record or handle prescribing, so a clinic that needs those is better off with a clinic system. Our aesthetics guide names some. Does Fresha have consent forms?
Yes. Fresha’s UK pricing page lists consultation forms and patch tests on both its plans. They come with the subscription, which is £17.94 a month with VAT for one person.
Are consultation forms included in Covered Beauty’s £9 plan?
Yes. Forms are on every plan.
Do my clients need an app to fill in a form?
Not on Covered Beauty. They open a private link on their phone, with no app or account.
Can I send a consultation form on WhatsApp?
Yes. Copy the private link into WhatsApp or a text. Don’t collect health answers as chat messages, because then they sit in your phone.
Are digital consultation forms legal in the UK?
Yes. A typed signature can be valid, and you still need to look after the answers properly. Your insurer may have its own requirements, so check with them.
