Are these consultation form templates free?
Yes. Each blank form below is a free A4 PDF to print, with no sign-up: the questions for that treatment with tick boxes and lines to write on, a line for your business name, the client’s consent to their health information being used, and signature lines for the client and for you. Each one has a guide saying what it asks and why. Check the wording with your insurer before you use it.
We make Covered Beauty, and these are our own ready-made forms, so we’ve said where they fit. Nothing here is legal or medical advice. Your insurer’s rules and the product manufacturer’s instructions come first.

There are 23 ready-made forms in Covered Beauty in all. The others cover injectables, teeth whitening, and ear and body piercing, with a general form for anything else. They’re in the app, ready to edit, but not here to print.
How do you make a template your own?
- Read it through first. Cross out what you don’t need and write in what you do, such as the products you use. The guide for each form suggests questions you might add.
- Check it with your insurer. Your policy sets what you have to ask and record. BABTAC’s Good Practice Guide, from 2016, is the trade body’s guidance for its members, and it says record cards should be signed by the client and by the therapist.
- Record the patch test. BABTAC’s guide lists lash and brow tinting, lash extensions, semi-permanent makeup, and lash perming and spray tanning where the manufacturer says so, among the treatments that need one. The forms that ask about a patch test have a record at the end for the date, the product, the result and who did it.
- Ask again every six months. BABTAC suggests reviewing a client’s personal details and medical history every six months.
- Keep them safe. A consultation form holds health information, which UK GDPR protects more strictly. What that means for you is in our guide to consent forms and UK GDPR.
Paper or online?
Paper works, but the client fills it in once they’re already in your chair, and the cards need locking away. The same forms are ready-made in Covered Beauty, where you can edit every word, and clients fill them in one of three ways:
- With their booking. Link a form to your treatments and it goes out with the confirmation, with a reminder 48 hours before if it isn’t done.
- A private link. Email it, or copy it into WhatsApp or a text. It works for 14 days and only for that client.
- On your phone or tablet. Set a kiosk PIN, then use Hand to client. It shows only their forms until someone from your team enters the PIN.
Every signed form lands on the client’s record, ready to print or download as a PDF. See how online consultation forms work. Try it free for a week, with no card needed. Forms are on both plans, from £9 a month.
Questions about the templates
Are these consultation form templates free?
Yes. Download and print them as often as you like, with no sign-up and no card. The foot of each page says it’s a free template from Covered Beauty.
Are the templates approved by insurers?
No. They’re starting points, not approved by any insurer or checked by a lawyer. Read each one through, add anything your treatments need and check it against your insurer’s requirements before you use it. If your insurer gives you approved wording, use that.
Can I change the wording?
Not on the PDF. The same forms are ready-made in Covered Beauty, where you can change every word, add or take out questions, and send them to clients online. You can try it free for a week, with no card needed.
Do I need a separate form for each treatment?
Our forms are one per treatment, so each asks only what that treatment needs. Pair them with a new client medical history form, which covers a client’s general health once, whatever they book.
How long should I keep signed consultation forms?
BABTAC’s 2016 Good Practice Guide tells its members to keep record cards for seven years, so check what your insurer asks for. Paper forms hold health information, so keep them locked away. Covered Beauty keeps signed forms for 7 years from the appointment, or from the day it was signed for a form filled in without a booking, while your account is open.
