Online consultation and consent forms,
signed before you start.

Send a client a private link to fill in on their own phone, or hand them your phone or tablet in the salon. Their signed form lands on their record, with or without a booking.

Can I use Covered Beauty just for consultation forms?

Yes. When you sign up, choose Consultation forms when we ask what you want first. Your Forms page then opens with the ready-made forms and three steps: pick a form, choose how clients fill it in, and get your first one filled in.

You don’t need treatments, opening hours or bookings for any of it. When you want clients to book online too, it’s already there on the same plan.

How do clients fill in their forms?

A private link to their phone.

Pick the client and the forms, up to 10 in one link. Email it from Covered Beauty, or copy it into WhatsApp or a text. It works for 14 days and only for that client, and you can cancel it at any time. Email it again and they get a fresh link, and the old one stops working. No app or account needed.

Your phone or tablet, in the salon.

Set a kiosk PIN, then use Hand to client: pick the client or add a new one, tick their forms and hand the device over. It shows only their forms, one after another, until someone from your team enters the PIN.

With every booking.

Link a form to the treatments it covers and it goes out with the booking confirmation, with a reminder 48 hours before if it isn’t done. Two treatments booked together? Each form comes up once, one after the other.

A form filled in without a booking, from a link or on your tablet, counts for any appointment the client has in the 30 days after, as long as you haven’t changed the form since. They’re never asked for it twice.

Which ready-made forms are there?

There are 22 ready-made forms, each a starting point you read through and make your own. They include:

  • Lashes and brows: lash extensions, lash lift, lash tint, brow tint, brow henna, brow lamination and brow wax
  • Nails, waxing and electrolysis
  • Tanning: spray tans and sunbeds
  • Skin: facials and skincare, dermaplaning and LED light therapy
  • Aesthetics: injectables, semi-permanent makeup and microblading
  • Massage and body treatments
  • Ear and body piercing
  • A new-client medical history form, and a general treatment form for anything else, such as hair extensions

Most open with a short note for the client on the treatment’s main risk. Every one asks for their name, date of birth and address, then the treatment’s own questions. The tint, henna, lamination and lash lift forms ask about a patch test. Forms such as sunbeds and injectables ask whether the client is under 18. Where aftercare matters there’s a tick to agree to it. Every form ends with a tick to confirm their answers, then their typed signature and the date.

Each one opens with a note saying it’s a starting point, not a finished or legally checked document. Read it through and check it against your insurer’s requirements before you switch it on. That note is just for you: clients never see it, and it isn’t printed on their signed form. Want to start from scratch? Build your own from short text, long text, dates, yes or no questions, tick boxes and information text.

What happens to a signed form?

  • It lands on the client’s record, and opens on its own page to print or download as a PDF.
  • That page shows when they signed, the name they typed, the version of the form they saw and their explicit consent.
  • Forms shows what’s been filled in recently and which links are still waiting.
  • Signed forms are kept for 7 years from the appointment, or from the day they were signed for a form filled in without a booking, while your account is open. If you ever leave, export them within 30 days of your account ending, as our terms say, so you still have them for your insurer.
  • Only team members you give the Forms permission can see the answers, in the dashboard or in a backup they download. You can add two-factor sign-in as well.
  • Databases and file storage are in the UK, in London, and you can export your data on both plans.

What about UK GDPR?

Answers about a client’s health are special category data, which UK GDPR protects more strictly. Every form shows the client a short note about their health information, and they can’t send it without ticking a separate box to give their explicit consent. We record when they ticked it.

You’re the controller of your clients’ data and we’re your processor, under our data processing agreement. Read our privacy notice and security page, and our guide to consent forms and UK GDPR for what it means for your salon.

What does it cost?

Forms are on both plans. Solo is £9 a month for one person, and Salon is £19 a month for up to five.

Online booking, deposits, reminders and client records come with it, for when you want them. Try it free for a week, with no card needed. See both plans.

What are the other ways to do consultation forms?

Paper record cards

They’re cheap to print, but they can go missing, they need locking away, and nobody can fill one in before they arrive. Moving from paper? Keep the cards you have for as long as your insurer asks, and start new forms online.

A general form builder

Jotform is a form builder for every kind of business, with consultation form templates among many others. Its free Starter plan allows 5 active forms, 100 submissions a month and 10 signed documents a month.

Consent form apps

Faces says it’s free to use, with no card needed, and has over 200 insurance-verified consent forms. It’s built for aesthetic clinics and healthcare practices, and lets them find prescribers and order from pharmacies alongside bookings.

Insync Consent, from Insync Insurance Solutions Ltd, is listed on the App Store as free and designed for iPad, with insurer-approved treatment consent forms, aftercare, a bookings calendar and a client directory.

Our ready-made forms aren’t approved by an insurer. If yours gives you approved wording, type it into a Covered Beauty form.

Booking systems with forms

Fresha lists consultation forms and patch tests among its features. Booksy includes custom forms and liability waivers. Phorest sends consultation forms before a visit from its Grow plan, and Timely from its Boost plan. For every option side by side, read the best salon booking software in the UK.

Questions about consultation forms

Can I send a consultation form without a booking?

Yes. Choose the client and the forms, then email the link from Covered Beauty or copy it into WhatsApp or a text. The link works for 14 days, only for that client, and you can cancel it at any time. Their signed form lands on their record.

Do clients need an app or an account?

No. They open the link on their phone, answer the questions, tick to give their consent and type their name to sign.

Can clients fill in a form on my iPad in the salon?

Yes. Set a kiosk PIN, then use Hand to client. The iPad shows only their forms until someone from your team enters the PIN.

Do returning clients have to fill in the whole form again?

If the form came with their booking and they filled in the same form in the last 6 months, they see their previous answers, confirm nothing has changed and sign again. Patch test questions never carry over, so they answer those again every time. Forms sent as a link or handed over on your tablet are filled in fresh each time, so nobody holding the device sees someone else’s answers.

Can I use my own wording or my insurer’s?

Yes. Edit any ready-made form word for word, or build your own from short text, long text, dates, yes or no questions, tick boxes and information text.

How long are signed forms kept?

7 years from the appointment, or from the day it was signed for a form filled in without a booking, while your account is open. You can print or download any signed form as a PDF, and export your data at any time. If you ever leave, export your forms within 30 days of your account ending, as our terms say, so you still have them for your insurer.

Is there a free trial?

Yes, a week free with no card needed. Forms are included on both plans, from £9 a month.
