Your lash lift consultation form,
question by question.

What to ask before a lash lift or tint, how to record the patch test, what to do if it stings, and a free blank form to print or send before they’re on your couch.

What should a lash lift consultation form include?

A lash lift consultation form should say who the client is, then ask whether they’ve had a patch test for the lift solution with no reaction, about allergies to the solution or the adhesive, eye conditions, recent eye surgery and contact lenses, and whether they’re pregnant or breastfeeding. It ends with the aftercare, their consent and a signature with the date. A tint the same day needs its own patch test question. Check the final wording with your insurer.

We make Covered Beauty, which has ready-made lash lift and lash tint forms, so we’ve said where they fit. Nothing here is legal or medical advice. Your insurer’s rules and your lift kit’s instructions come first.

What does our lash lift form ask?

Every ready-made form in Covered Beauty starts with the client’s full name, date of birth and home address, so the signed form stands on its own as a record. Then the lash lift form asks:

  • Have you had a patch test for this solution within the last 6 months, with no reaction? (yes or no)
  • Any known allergy to lash lift solution or adhesive? (yes or no)
  • Any eye conditions, recent eye surgery, or do you wear contact lenses? (yes or no)
  • Are you pregnant or breastfeeding? (yes or no)
  • I understand the aftercare advice given and agree to follow it. (a tick to agree)
  • I confirm the information I’ve given above is accurate, and I consent to this treatment. (a tick to agree)

Its note to the client says the solution goes on right next to the eye, so irritation or a reaction there is the main risk, and asks them to tell you straight away if their eyes sting or water more than expected.

Why does each question matter?

The patch test

BABTAC’s Good Practice Guide, from 2016, is the trade body’s guidance for its members. It lists eyelash perming among the treatments that need a patch test where the manufacturer says so. The national occupational standard for lash lifts and brow lamination asks you to carry out the tests the manufacturer’s instructions give, whether that’s a skin test, a sensitivity test or an allergy alert test, and to record the outcome. It isn’t law, but qualifications are built on standards like it.

The solution and the adhesive

A lift uses an adhesive to hold the lashes on the shields, then a lifting solution and a fixing solution. The standard describes how they break the bonds in the hair and set them again in the new shape. That’s why the form asks about allergies to the solution and the adhesive, and not just to the solution.

Eyes, surgery and contact lenses

The standard’s list of adverse reactions includes eye irritation, damage to the eye and lash loss. If the client wears contact lenses, ask them to bring their glasses or a case, so the lenses can come out before you start.

Pregnancy

BABTAC’s guide says to patch test again after hormonal changes such as pregnancy or menopause, so a client who was fine last year may need a fresh test now.

What our form doesn’t ask

The standard says to find out the client’s treatment history, including that henna and perming chemicals don’t mix and can damage the hair. Our form doesn’t ask about it, so you might add questions like these:

  • Have you had a lash lift, tint, henna or lash extensions in the last 8 weeks?
  • Are your eyes sore, red or watery today, or do you have a stye or an eye infection?
  • Have you had a reaction to a lash or brow treatment before?

Change the time in the first question to match your lift kit’s instructions.

Doing a tint in the same appointment?

A tint is a dye, with its own patch test and its own allergies. Our lash tint form asks:

  • Have you had a patch test for this tint within the last 6 months, with no reaction? (yes or no)
  • Any known allergy to hair dye or tint (including PPD)? (yes or no)
  • Any eye conditions, recent eye surgery, or do you wear contact lenses? (yes or no)
  • I confirm the information I’ve given above is accurate, and I consent to this treatment. (a tick to agree)

It has no aftercare tick, so add your tint aftercare as information on the form if you give any. A client booking a lift and a tint together, with a form linked to each, fills in both forms, one after the other, and a form both treatments share comes up once.

How do you record the patch test?

BABTAC’s guide says to:

  • patch test at least 24 hours before treating a client for the first time, or 48 hours before for clients with diabetes or other special circumstances
  • test again after a change in their medical history, a new product or formula, hormonal changes, or 12 months since their last treatment
  • follow the manufacturer’s instructions if they ask for testing more often
  • never go ahead with a client who reacted to the patch test

To the question of whether you still need to patch test after years of tinting and perming lashes with no problems, the guide’s answer is definitely yes. It also says a consent form a client signs to skip a patch test the treatment needs won’t protect you.

  • Our lash lift and lash tint forms ask whether the client has had a patch test in the last 6 months with no reaction. Change the wording to match your insurer and the product.
  • Patch test questions are asked fresh at every visit, never carried over from last time.
  • Add a patch test as its own short treatment with no deposit, so clients can book one online before their appointment.
  • Covered Beauty doesn’t stop a client booking a lift without a patch test, so check their form before you start.

What if their eyes sting during the lift?

BABTAC’s guide says to protect the client’s clothes, turn their head to the side of the eye that’s stinging, and bathe the eye with warm water and clean cotton wool, letting a stream of water run from the tear duct to the corner of the eye and back into the bowl. Ask them to keep their eyes closed, bathe rather than rub, and keep going until the burning settles.

Our form’s note asks clients to tell you straight away if their eyes sting or water more than expected, so they know before you start. Write anything that happened in the client’s notes.

How do you get it signed before they arrive?

Covered Beauty gives you three ways to have it done before you start:

  • With their booking. Link the form to your lift and tint treatments and it goes out with the confirmation, with a reminder 48 hours before if it isn’t done.
  • A private link. Email it, or copy it into WhatsApp or a text. It works for 14 days and only for that client.
  • On your phone or tablet. Set a kiosk PIN, then use Hand to client. It shows only their forms until someone from your team enters the PIN.

When the form comes with a booking and the client filled in the same form in the last 6 months, they see their previous answers and confirm nothing has changed, then sign again, but the patch test question is asked afresh. Forms sent as a private link or handed over on your tablet are always filled in fresh, so whoever holds the device never sees another client’s earlier answers.

The signed form lands on the client’s record, ready to print or download as a PDF. Covered Beauty keeps it for 7 years from the appointment, or from the day it was signed for a form filled in without a booking, while your account is open. If you ever leave, our terms give you 30 days after your account ends to export your data. See how online consultation forms work. Try the lash lift and tint forms free for a week, with no card needed. Forms are on both plans, from £9 a month.

Each one is an A4 PDF with every question that form asks (listed above), boxes to tick yes or no, a line for your business name, and lines for the client’s signature and yours with the date. The extra questions we suggest aren’t on them, so write in the ones you need. The forms that ask about a patch test end with a patch test record for you to fill in. The foot of each page says they’re free templates from Covered Beauty. Print them, keep a few on a clipboard, and check the wording with your insurer first.

Paper still means the client fills it in once they’re in your room. Send this form before they arrive, free for a week, with no card needed.

Questions from lash techs

Do you need a patch test before a lash lift?

BABTAC’s 2016 Good Practice Guide lists eyelash perming among the treatments that need a patch test where the manufacturer says so. The national occupational standard for lash lifts asks you to carry out the tests the manufacturer’s instructions give and record the result. Check your lift kit’s instructions and your insurer’s rules.

Can I do a lash lift and a tint in the same appointment?

If you offer both together, each product needs its own patch test, so ask about both, and follow both manufacturers’ instructions on the order and timing. In Covered Beauty a client booking both fills in the lash lift form and the lash tint form, one after the other.

Should a lash lift form ask about pregnancy?

Our form asks whether the client is pregnant or breastfeeding, so you can talk it through with them. BABTAC’s 2016 Good Practice Guide says to patch test again after hormonal changes such as pregnancy or menopause. Check your lift kit’s instructions and your insurer’s rules before you go ahead.

Can I send a lash lift consultation form on WhatsApp?

Yes. In Covered Beauty, choose the client and the form, then copy the private link into WhatsApp. They fill it in on their own phone, the link works for 14 days and only for that client, and the signed form lands on their record. The answers stay out of your chat.

Is there a free lash lift consent form template?

Yes. The blank lash lift and lash tint forms on this page are free to download and print, with no sign-up. You can also try the same forms in Covered Beauty free for a week, edit every word and send them before the appointment.
