Your spray tan consultation form,
question by question.

What to ask before a spray tan, the patch test and timing to check, what to tell clients before they come, and a free blank form to print or send ahead.

What should a spray tan consultation form include?

A spray tan consultation form should say who the client is, then ask about allergies to DHA or self tan, asthma or any breathing condition, pregnancy or breastfeeding, and open cuts, sunburn or a skin condition today. Record the patch test the tan maker asks for, check they haven’t just waxed and have no laser or IPL coming up, give the aftercare, and end with their agreement to it, their consent and a signature with the date. Check the final wording with your insurer.

We make Covered Beauty, which has a ready-made spray tan form, so we’ve said where it fits. Nothing here is legal or medical advice. Your insurer’s rules and your tan maker’s instructions come first.

What does our spray tan form ask?

Every ready-made form in Covered Beauty starts with the client’s full name, date of birth and home address, so the signed form stands on its own as a record. Then the spray tan form asks:

  • Any known allergy to DHA or self-tan products? (yes or no)
  • Do you have asthma or any breathing condition? (the tan is applied as a fine mist) (yes or no)
  • Are you pregnant or breastfeeding? (yes or no)
  • Any open cuts, sunburn, or active skin condition today? (yes or no)
  • I understand the aftercare advice given and agree to follow it. (a tick to agree)
  • I confirm the information I’ve given above is accurate, and I consent to this treatment. (a tick to agree)

Its note to the client says the tan goes on as a fine mist, so breathing it in is the main thing to be careful of with asthma or another breathing condition, and that the solution can sting on broken skin. Before the aftercare tick, it tells them to avoid water, sweating and tight clothing for as long as you advise, and to moisturise daily.

Why does each question matter?

Allergies and the patch test

BABTAC’s Good Practice Guide, from 2016, is the trade body’s guidance for its members. It lists spray tanning among the treatments that need a patch test where the manufacturer says so, and its tanning advice starts with one. The national occupational standard for manual and spray tanning asks for a test of skin sensitivity before the treatment, and covers the active ingredient, DHA, and what it does to the skin. It isn’t law, but qualifications are built on standards like it.

Our form asks about allergies to DHA or self tan, but not about a patch test, so add a yes or no question such as “Have you had a patch test with our tanning solution, with no reaction?” Any question that mentions a patch test is asked fresh at every visit, never carried over from last time.

Asthma and breathing

BABTAC’s guide says the room needs good ventilation, that you should warn the client the air from the spray gun may take their breath away slightly, and that without proper extraction and filtration both of you should wear a mask. The standard lists watery eyes, coughing and fainting among the adverse reactions. Asking about asthma first lets you plan for it.

Pregnancy

BABTAC’s guide says hormonal changes in pregnancy may affect the result, and that, as with all body treatments, the treatment mustn’t be done in the first three months. Our form asks whether the client is pregnant or breastfeeding, so you can follow your insurer’s rules.

Skin today

The form asks about open cuts, sunburn or a skin condition on the day, because the solution stings on open skin. BABTAC’s guide suggests an oil-free moisturiser on dry areas so the tan doesn’t grab, which can also be used on clients with psoriasis or eczema.

What our form doesn’t ask

BABTAC’s guide says not to spray tan straight after waxing or other hair removal, because the tan collects in the open pores, nor in the 10 days before IPL or laser, because it can cause blistering and irritation. You might add:

  • Have you had a patch test with our tanning solution, with no reaction?
  • Have you waxed or had any other hair removal in the last 24 hours?
  • Do you have laser or IPL booked in the next 10 days?
  • Have you used any other tanning product in the last 7 days?

What should clients do before they come?

BABTAC’s guide says to tell clients when they book to:

  • exfoliate and moisturise regularly before the tan
  • wear loose, dark clothing
  • remove unwanted body hair at least 24 hours before
  • stop using any other tanning product seven days before
  • wear no make-up, deodorant, perfume, body oils or lotions on the day

In Covered Beauty, put this in the treatment’s description, which clients read when they book, and send your aftercare from the Aftercare page once they’ve been in.

How do you get it signed before they arrive?

Covered Beauty gives you three ways to have it done before you start:

  • With their booking. Link the form to your tanning treatments and it goes out with the confirmation, with a reminder 48 hours before if it isn’t done.
  • A private link. Email it, or copy it into WhatsApp or a text. It works for 14 days and only for that client.
  • On your phone or tablet. Set a kiosk PIN, then use Hand to client. It shows only their forms until someone from your team enters the PIN.

Regulars back every week or two don’t fill it all in again: when the form comes with a booking and they filled in the same form in the last 6 months, they see their previous answers and confirm nothing has changed, then sign again. If something has changed, they fill in the whole form afresh. Forms sent as a private link or handed over on your tablet are always filled in fresh.

The signed form lands on the client’s record, ready to print or download as a PDF. Covered Beauty keeps it for 7 years from the appointment, or from the day it was signed for a form filled in without a booking, while your account is open. If you ever leave, our terms give you 30 days after your account ends to export your data. See how online consultation forms work. Try the spray tan form free for a week, with no card needed. Forms are on both plans, from £9 a month.

It’s an A4 PDF with every question our form asks (listed above), boxes to tick yes or no, a line for your business name, and lines for the client’s signature and yours with the date. The extra questions we suggest aren’t on it, so write in the ones you need. The foot of each page says it’s a free template from Covered Beauty. Print it, keep a few on a clipboard, and check the wording with your insurer first.

Paper still means the client fills it in once they’re in your room. Send this form before they arrive, free for a week, with no card needed.

Questions from tanning pros

Do you need a patch test before a spray tan?

BABTAC’s 2016 Good Practice Guide lists spray tanning among the treatments that need a patch test where the manufacturer says so, and its tanning advice starts with one. The national occupational standard for spray tanning asks for a test of skin sensitivity before the treatment. Our form doesn’t ask about a patch test, so add a yes or no question about one.

Can you spray tan someone with asthma?

Our form asks about asthma and other breathing conditions because the tan goes on as a fine mist, so you can talk it through first. BABTAC’s 2016 Good Practice Guide says the room needs good ventilation, and that without proper extraction and filtration both you and the client should wear a mask. Check your tan maker’s instructions and your insurer’s rules.

Can you have a spray tan when pregnant?

BABTAC’s 2016 Good Practice Guide says hormonal changes in pregnancy may affect the result, and that, as with all body treatments, the treatment mustn’t be done in the first three months of pregnancy. Our form asks whether the client is pregnant or breastfeeding. Check your insurer’s rules too.

How long after waxing can you have a spray tan?

BABTAC’s 2016 Good Practice Guide says not to spray tan straight after waxing or other hair removal, because the tan collects in the open pores, and to remove unwanted hair at least 24 hours before. It also says not to spray tan in the 10 days before IPL or laser.

Is there a free spray tan consent form template?

Yes. The blank spray tan form on this page is free to download and print, with no sign-up. You can also try the same form in Covered Beauty free for a week, edit every word and send it before the appointment.
